SCHOLASTIC AWARD APPLICATION FORM

Interested students should complete the “Application for Bonita Optimist Club Foundation Scholastic Award“, and provide the following: three character reference letters (must include at least 2 letters from educators), a biographical one-page essay, proof of community service, and an official transcript of courses completed. Application and supporting documents must be submitted by April 30th.
